What is Idaho Medicaid Program?

The Idaho Medicaid State Plan is made up of the Standard State Plan which includes mandatory minimum benefits and three Benchmark Benefit plans that are aligned with the health needs of Idahoans and includes an emphasis on prevention and wellness.

Who is Eligible?

In order to receive health coverage from Idaho Medicaid, you must complete the application process and meet certain eligibility requirements. You must fit one of the following categories:

  • Be a child under the age of 19, or
  • Be a pregnant woman, or
  • Be an adult with a child under the age of 19, or
  • Be age 65 or older, or
  • Be blind or disabled according to Social Security Administration criteria.
If you fit one of the categories above, you must also meet the following eligibility criteria:
  • You must be a citizen or legal immigrant, and
  • You must be a resident of the State of Idaho, and
  • Your household income must be less than the program income limits for your household size, and
  • Your resources must not exceed the program resource limits.

  • EPSDT Services for children under the age of 21 (must be prior authorized)

  • Medical equipment and supplies
  • Medical Transportation services
  • Nurse Midwife
  • Pregnancy and family planning services
  • Prescriptions
  • Primary Care Case Management
  • Prosthetics/orthotics
  • Substance Abuse Treatment
  • Smoking Cessation
  • Vision Services
  • Weight loss
  • X-rays

Additional services, such as nursing homes or personal care services, may be available to those who have special needs.

How to Apply?

You can apply for Medicaid with just your name, address and signature on an application for assistance, or you may use any of the following options to apply for Medicaid and other Health Coverage Assistance options:

Where can I report a change? What changes do I need to report?

If your income, resources, living arrangements, family size, or other circumstances change, it can affect your eligibility. Each program has different reporting requirements. It's your responsibility to let your local Health and Welfare office know about these changes. If you have private health care insurance and your coverage under that policy changes, you need to let your local Health and Welfare office know.

Will I have to pay co-payments?

Medicaid providers might charge a co-payment for some routine, non-emergency services. These include:

  • Using the emergency room when it's not an emergency
  • Using emergency medical transportation when it's not an emergency
  • Chiropractic care
  • Occupational therapy?Optometry
  • Physical therapy
  • Podiatry
  • Speech therapy
  • Doctor visits

Contracted Health Plans

The Basic Planis for low-income children and adults with eligible dependent children. This plan provides health, prevention, and wellness benefits for children and adults who don't have special health needs. Most participants will be in this benefit plan.
The Enhanced Planis for individuals with disabilities or special health needs. This plan has all the benefits of the Basic Plan, plus additional benefits.
The Medicare-Medicaid Coordinated Plan is for individuals who are eligible for both Medicaid and Medicare.
